The Queen’s Health System Website Privacy Policy

1. Introduction

This Privacy Policy (“Policy”) describes how The Queen’s Health System (“QHS,” “us,” “our,” or “we”) collects, uses, and discloses personal information from or about you through the websites we maintain that link to this Policy, including appointment.dlslab.com (the “Site”).

This Policy does not apply to other online services, such as websites and mobile applications, that we own, provide, or maintain, but do not post a link to this Policy.

This Policy also does not apply to protected health information that we collect from individuals pursuant to the Health Insurance Portability and Accountability Act. Information regarding our privacy practices with respect to protected health information under HIPAA can be found in the Notice of Privacy Practices provided to you at the time of collection.

2. What Personal Information is Collected

We may obtain personal information through the Site, such as when you register a Community Investment application account, donate money to QHS, or register for events. Personal information we collect will vary depending on your interactions with us. Personal information may include contact information (such as name, email address, postal address, and phone number), business contact information (such as your employer and title), and payment and billing information (such as when you make a donation).

3. Cookies and Other Similar Technologies

We or our vendors may use cookies, web beacons, pixels, and other similar technologies on certain webpages or locations of the Site for various purposes, including to provide functionality of the Site, to improve performance of the Site, to personalize your interactions with us, and to analyze and better understand how the Site is being used. We may implement data modification and obfuscation tools to help us limit how certain information may be collected from vendors through online tracking technologies. These tools act as an intermediary between the Site and cookies and similar technologies, and allow us to adjust how such technologies interact with our Site.
